If you have a plan with one of these providers, you may be able to use your insurance benefits to cover some—or even all—of your massage therapy sessions.

What You’ll Need to Get Started
Before we can book your session, we’ll need a few important things on file:
- Insurance Intake Form – This form collects your insurance information and a written doctor’s referral or prescription.
- Doctor’s Referral/Prescription – Depending on your insurance, a referral may be required.
- This referral should include ICD-10 codes (diagnostic codes) that specify the condition or area of the body needing treatment.
- Referrals can be faxed to us at (801) 697-6510.
Specific Guidelines by Insurance Provider
TriWest Insurance
- Coverage must come through the Community Care program.
- A referral with an authorization number is required and must list the number of approved visits.
- ICD-10 codes are mandatory for TriWest coverage—we cannot provide services without this.
- A referral/prescription is not required, but we highly recommend getting one with specific ICD-10 codes to ensure the best care.
- These codes help guide our therapists in working on exactly the areas that your doctor has identified.
